The consultation form asks for contact information, city or ZIP code, an estimated due month, general area of interest, and a brief message. Do not submit diagnoses, test results, insurance identifiers, Social Security numbers, payment information, or urgent symptoms.
How information is used
Information is used to respond to your inquiry, arrange a consultation, and assess basic location and schedule availability. It should not be treated as a medical record, diagnosis, patient-provider relationship, or emergency communication.
Form processing and hosting
The website's hosting and form service may process technical information such as IP address, browser type, timestamps, and spam signals. The consultation form is limited to basic contact and scheduling information.
Analytics and cookies
The website contains no advertising pixels, behavioral analytics, embedded social feeds, or nonessential cookies. The OpenStreetMap frame on the contact page loads only when that page is visited and may receive ordinary connection information.
Clinical information
Julia provides current clinical consent and disclosure forms directly by individual email or at the relevant visit. Follow the return instructions supplied with each form. Do not send signed forms, records, test results, or sensitive health information through the public consultation form.
Retention and requests
